Highly sensitized multiorgan transplant candidates are challenging for the transplant providers. Currently, there is no consensus on assessment of these patients' immunological risk, waitlist management or post-transplant immunosuppression. Growing evidence suggests liver allografts provide immunological protection to the simultaneously transplanted organs through different mechanisms. This course will review the most current clinical and experimental data, and identify areas for further research and allocation policies that will help transplant this difficult cohort of patients.
